Introduction: Morrison Insurance Services (“Sponsor”) is offering a Referral Rewards Incentive Card program(the “Promotion”)

Eligibility: Promotion open by Sponsor invitation only. Void where prohibited by law or regulation. The Promotion is subject to all applicable federal, state and local laws. Any entries not eligible for any reason will be disqualified from collecting a reward and ineligible entries may be deleted. Any entry that is awarded funds and is later determined to have been ineligible will be required to return any award of monetary or material value within 7 days upon notification of disqualification and ineligibility. Employees of the Sponsor and its subsidiaries, affiliated companies, agents, and their immediate family (defined as parents, spouse, children, siblings, grandparents), and all those with whom such employees are domiciled, are NOT eligible to participate in the Promotion.
Entry Requirements: Sponsor invitation required for entry. Referral submissions do not qualify entrant for rewards, qualification dependent on quoted referrals. Each quoted referral will count as one eligible reward. To enter the Promotion, visit http://www.morrison-insurance .com/entry-form. Submit all required information. By clicking ‘Submit’ entrant agrees to Promotion Terms and Conditions.
All Entries must be submitted between 12:00:01 a.m. Pacific Standard Time (“PST”) on January 1, 2015, and 11:59:59 p.m. PST on December 31, 2015 (the “Entry Period”). All referrals must be quoted before 11:59:50 p.m. PST on January 6, 2016 for entry eligibility. After entrants are awarded there reward funds, the entrants will make themselves available for written / over-the-phone interviews by Sponsor and its designees.
Winner Notification: Limit one prize per entrant. The winners will be notified via email and/or phone. Potential winners must respond to their notification within 7 days of receipt or they will be disqualified. Potential winners may be required to execute and return and Affidavit of Eligibility, Liability and (where legal) Publicity Release within 14 days of issuance of notification or they will be disqualified. Noncompliance with the foregoing, these official rules, Sponsor’s inability to contact potential winners within a reasonable time period or notification returned as non-deliverable, may result in disqualification and, at Sponsor’s discretion, a runner-up may be notified.
By accepting any funds, the entrant grants Sponsor and its designees the right to use his/her name, city and state of residence, likeness and biographical information in any and all media for any purposes throughout the world in perpetuity, without additional compensation, notice or permission, unless prohibited by law. If a entrant requests (in writing to Sponsor) confidentiality, only a first name and state will be provided.
Prize: One grand-prize winner and two runner-up winners will be awarded. Grand-prize winner will receive and Apple iPad valued at $499.00 US. Runner-up winners will receive either (1) 2 one-day lift tickets to Northstar California Resort valued at $182.00 US, or (2) a $100.00 gift certificate to the Atlantis Resort & Casino Steakhouse and 2 movie tickets, package valued at $119.00 US. Winner pays all related taxes and fees. Awards subject to change at Sponsor’s discretion.
Limitation of Liability: Sponsor and its affiliates will not be responsible for late, lost, stolen, or misdirected, damaged, incomplete, or illegible entries; or entries that are lost due to computer, Internet, or electronic malfunction. Sponsor reserves the right to cancel or modify the Promotion and award the funds through an alternate means if fraud or technical failure compromises the integrity of the Promotion, as determined by it in its sole discretion. Entrants agree that Sponsor, its promotion partners, affiliates, agencies, and their respective employees or officers shall not be liable for injury, losses, damages, or costs of any kind resulting from participation in this Promotion or acceptance of or us or misuse of funds or parts thereof. In the even to a dispute regarding the identity of the person submitting an electronic entry will be deemed to be submitted by the person in whose name the online form is registered.
Governing Law; Venue: This Promotion will be governed by the laws of the State of Nevada without reference to its choice of law principles. The entrants agree to submit to the exclusive jurisdiction of the state courts located in Washoe County in the State of Nevada, with respect to disputes hereunder.
